What are VAC Packets?
- The VAC packet is a part of the onboarding process to get you ready for your internship. It is a packet of documents that includes your emergency contact information, VMTH volunteer permit forms, and waiver forms which the VMTH must have on record at all times. Additionally, it includes all your safety training transcripts, a description of VMTH safety analysis, and an orientation quiz. Before you attend your first shift at the VMTH, this packet must be printed out and handed to an officer to initial.
- The packet will be sent to you via an email. All you need to do is download the packet as a pdf, print it out, and complete it using blue or black ink ONLY.
- Go through the packet page by page and fill out the highlighted sections only. Where it says “speciality”, name the department you will be interning with.
- You must complete ACU 101, VMTH Biowaste, VMTH EAP & IIPP, and VMTH Laser Safety training (the refresher course). All of these will be emailed to you except ACU 101. In the event that you don’t receive an email to complete the VMTH specific training, login into “UC Davis LMS” and your training should show up. If it is still not visible, please contact Amrita ([email protected]). In order to access ACU 101, please visit this LINK and they will unlock the portal for you.
- The JSA packet is a packet explaining different clinical areas you will encounter in the VMTH along with an SOP of how to conduct certain operations. This packet needs to be signed on the first page and turned in with your VAC Packet.
- The orientation quiz is a powerpoint that will be attached in the email along with your VAC packets. Go through the powerpoint and take the orientation powerpoint quiz that is attached to the packet.
- ALL CONTENTS OF THE PACKET NEED TO BE PRINTED OUT (except the orientation ppt). An email will be sent with a checklist.
- Packets can be turned in through the drop box in South Hall, or they can be submitted in-person. Office hours will be held two to three times this quarter either at the MU or Silo where you can turn your packets in to our VP, Amrita, directly. Emails will be sent out with dates and time soon!
